The government must give individuals notice when collecting private or confidential information from them. This is referred to as a "Tennessen Warning Notice." Government may also call it a "Privacy Notice," a "Notice of Collection of Private/Confidential Data," or something similar. The purpose of the notice is to enable people to make informed decisions about whether to give information about themselves to the government. (See Minnesota Statutes, section 13.04, subdivision 2.) This notice will be provided anytime we collect private data and you will be required to acknowledge receiving it.
This form must be completed by each individual applying for a permit or license requiring a background check. The consent is valid for 90 days.
Minnesota State Statute 176.182 requires evidence of compliance with workers' compensation insurance coverage. All applicants must complete this form.
Minnesota State Statute 270C.72, Chisago County is required to provide the Minnesota Commissioner of Revenue your business tax identification number and/or Social Security Number for each licensed applicant.
